IS and SA forms are dynamic PDF forms. To use, open the form in Adobe Acrobat Reader and fill in the appropriate fields. Then print the form by using the "Print Form" button on the form itself, clicking File --> Print from your browser, or CTRL-P from your keyboard. You may also save the completed form to your hard drive. If you need to use the form multiple times, you can press the "Reset From" button on the form to clear all fields.
